Consumer-Directed Services Facilitation for eligible CCC Plus members.
The CCC Plus Waiver is a Virginia Medicaid Home and Community-Based Services waiver serving eligible older adults, individuals with physical disabilities, and individuals who are chronically ill or severely impaired and require substantial and ongoing skilled nursing care.
For eligible CCC Plus members, certain services can be delivered through either an agency-directed or consumer-directed model. Under Consumer Direction, the member or designated representative takes responsibility for applicable employer functions.
Depending on individual eligibility, assessment, authorization, and applicable Medicaid requirements, Consumer-Directed services may include:
Authorized assistance with eligible activities of daily living and other covered personal care needs.
Temporary support intended to provide relief to an unpaid primary caregiver when authorized under the member’s plan.
Virginia Medicaid identifies Consumer-Directed personal care and respite as available Consumer-Directed services under the CCC Plus Waiver.
Caring Home Health Services LLC serves as the Services Facilitator—not the employer of your attendant.
We help you understand the responsibilities associated with Consumer Direction and provide ongoing facilitation according to applicable Medicaid requirements.
